LVN Intake Coordinator - Wharton

Performing the new patient intake procedures for home health admission.
Accept and process all referrals, identify payer sources, verify benefit information and acquire initial authorization as needed to begin patient service or to determine denial of patient service.
Follows up and maintains pending unresolved referrals and coverage issues in a timely manner.
Functions effectively as part of an exceptional home health team to deliver outstanding customer service to both internal and external customers, patients, and clients.
LPN/LVN with unrestricted license in the state of agency.
A minimum of one year experience in a telephonic triage/intake/disease management environment.
Strong customer service and problem solving skills.
Strong data entry skills with minimal errors.
Effective communication skills and the ability to interact with individuals from various scopes of the home health industry.
Excellent verbal and written communication skills.
